Answer:

NHS Ethical review is required for projects involving NHS patients, their personal data or their tissue, adults lacking the capacity to consent for themselves, a licensed or unlicensed drug, a borderline product (e.g. herbal supplement); a medical device or human tissue.

For further information on NHS ethical review see: 

If your project does not involve any of the above but involves human participants or their data or has a risk of significant environmental impact then University ethical review is required.
